Afterward, we'll explain our findings, answer any questions you may have, and recommend a personalized plan of care designed to help you achieve lasting results.

A valid ID
Insurance information (if applicable)
Any relevant medical records or imaging (if available)
Completed new patient paperwork
If you haven’t completed your forms yet, no problem—we’ll have them available in-office as well.
❋ Insurance and PaymentWe accept most major insurance plans and are happy to verify your benefits before your visit. Our team will walk you through what is covered and clearly explain any out-of-pocket costs ahead of time.
We also offer flexible, per-visit payment options to keep care simple and accessible. You will never be required to pay for an entire treatment plan upfront.
